What is Functional Medicine

Functional medicine is an approach to health that focuses on identifying the underlying Root causes of disease and illness rather than addressing isolated set of symptoms. It is personalized medicine. It represents a shift from the traditional disease-centered approach to a patient-centered approach.

How is Functional consulting visit different from Traditional Medical visit?

Expanded evaluation time from 90-120 minutes to compile a comprehensive patient history and perform an in-depth physical exam. Traditional medical practice covered by insurance allows limited evaluation of 12-20 minutes with brief physical exam.

Scope of evaluation – Systems biology or root cause evaluation from expanded evaluation to uncover associated patterns and physiology underlying the individual’s current health. Traditional medical evaluation focuses only on recognized conditions, disease focused evaluation.

Testing – Use of advanced testing and analysis which typically require 45-60 minutes to analyze – see the list of tests. Traditional practice uses Standard of Care testing as determined by ICD 10 codes and approved by Insurance company.

Expanded Treatment options – Investigation of Novel treatments for diseases, based on patient’s unique situation, including Neutraceuticals, Herbiceuticals, and alternative treatment modalities.

Expanded follow up time of 45-60 minutes to review extensive lab testing and formulate your individualized treatment plan.

Integrative and Holistic Specialists: Consultation with other integrative specialists based on patient’s response to therapy or lab testing.

GI Related Issues such as uncomfortable bloating, chronic abdominal pain, IBS, Acid reflux
Conventional approach – 

Patients are briefly counseled on diet and lifestyle to prevent acid reflux, start a trial on medications called PPIs such as Omeprazole to block stomach acid. If no improvement will be referred to GI specialist who usually does routine procedures such as upper endoscopy, H pylori testing, colonoscopy and biopsies for any lesions found. These protocols can cure some patients, but many return to primary care with no improvement.

The conventional approach to treating heartburn and gastro esophageal reflux disease, or GERD, involves using several drugs that may offer some temporary relief. Acid-suppressing drugs like proton pump inhibitors (PPIs) aren’t a permanent solution—and they carry risks of their own.

Functional approach –

If you want real relief from your heartburn or GERD symptoms, you need to treat the underlying cause. In many cases that’s low stomach acid—no matter how counterintuitive that sounds.

Thyroid Disorder :

20 million Americans suffer from thyroid disorder. Majority of people with thyroid disorders are not even aware they have a problem. Symptoms of low thyroid function such as dry skin, weight gain or loss, mental slowing, hair loss and irregular periods, some cases even fertility issues.

Conventional approach: 

Thyroid supplements, usually T4 replacements. Many patients continue to have symptoms as levothyroxine replacement does not address mineral deficiencies, autoimmunity, the most common root causes of the thyroid disorder. Another common condition that is missed is low T3 syndrome. In this case the issue lies with how body regulates thyroid metabolism, not with the thyroid gland itself.

Functional approach: 

Addresses Root causes of thyroid disorder such as autoimmunity, Mineral deficiencies, low T3 syndrome.

Diabetes:

Conventional approach: 

Once a patient is diagnosed with diabetes ( Fasting plasma blood sugar of >126 on at least two occasions, or a single hba1c of >6.5), then first-line medication metformin is started, diet and lifestyle. If adequate control not reached then second line medication such as Januvia, Onglyza, Victoza, Jardiance are begun. If these don’t control, ultimately insulin is used.

Integrative approach: In addition to the above, mineral and nutritional deficiencies are addressed, herbal supplements such as Berberine, and traditional remedies like ACV, Cinnamon, Curcumin are used to help with insulin resistance.

Functional Medicine approach: 

FM would address all above and also would include a systems biology approach which is different for each individual based on their evaluation. According to most recent literature, insulin resistance is a spectrum in which Hba1c over 5.5 or fasting sugar over 100 are markers of insulin resistance and treatment would begin at this stage rather than wait until the A1c is at 6.5. 

Diabetes is a part of a larger spectrum of diseases commonly called Metabolic Syndrome. For example, one may have detoxification issues resulting in systemic vascular inflammation and insulin resistance. Or another could have pancreatic or biliary insufficiency resulting in incomplete food digestion and inadequate nutritional absorption which can affect blood sugar regulation. Someone else could have an immunologic reaction with a certain food like gluten causing vascular inflammation and insulin resistance and a fourth person could have all the above. Function Medicine would address these.
