Dr. Sailaja Reddy works as a functional medicine consultant in conjunction with your healthcare team and is not considered a replacement for your primary care physician. She requires all new patients to have a primary care physician, to cover emergencies and routine healthcare screening.
It is important to submit the forms at least 7 days prior to your appointment. Having these forms 7 days in advance will allow Dr. Reddy to help solve your problems more efficiently and enhance the quality of your care. If your Intake Forms and Medical Records have not been received at least 7 days prior to your initial appointment, it may take Dr. Reddy up to 30 minutes of your appointment time to review your chart.

MEDICAL RECORDS FROM OTHER DOCTORS/CLINICS/HOSPITALS
Medical records can only be released with your authorization. It is your responsibility to obtain previous medical records from other physicians, or health care providers that you wish Dr. Reddy to review. Please contact your physician or other health care provider to obtain these records and make sure that we have received them at least 7 days prior to your initial appointment.

If you need copies of your medical records; a $30-$50 fees will be charged for copies and postage.
FUNCTIONAL MEDICINE CONSULTATION FEES
Please check our website appointment packages and choose which one fits your needs. After Dr. Reddy reviews your case and finds your case needs a different package than what you chose, we will inform you about this change prior to the appointment. Genetic testing requires an additional fee.
Please contact the office for specifics regarding visit fees. Additional costs may be incurred for testing, medication, and supplements. Depending on your insurance, some or all of these costs may be reimbursed. Please call the office to discuss with our staff if you have any questions about the financial aspects of the practice. We encourage you to directly contact your insurer to determine if your visits and medications and testing will be covered.
We are Membership based practice. In exchange for the membership described in your Membership agreement, Member agrees to pay RootHealthMD, the amount as set forth in your membership agreement, based on the level of membership reship that is mutually agreed upon. Payment is processed with automatic monthly membership billing. Please check in our website under Membership information.
Please contact the office for specifics regarding Membership fees. Additional costs may be incurred for testing, medication and supplements. Your Membership fee is not reimbursed by any medical insurance including Medicare or Medicaid. it is a self-pay fee. Many HSA and FSA accounts cover medical services, such as functional medicine It is up to you, as the Member, to check your specific HSA/FSA account. We neither submit the patient’s medical notes to insurance companies, not assist you with insurance claims.
We do not have a phlebotomy service at our location. We send our patients to a nearby lab. Your lab test may not happen the same day as we are required to schedule an appointment with the blood drawing lab in advance. If you think having a lab test done on a different day would be inconvenient, please let Dr. Reddy know via the portal, so we can arrange for the blood draw the same day of the appointment.
All of the supplements that are recommended are ordered through an online dispensary.

FOLLOW UP APPOINTMENTS
At the time of check out, you will be scheduled for a follow-up appointment. We will assume you will honor this appointment time unless you notify us otherwise at least 48 hours/ 2 business days prior to your scheduled appointment.
Cash, checks, or credit cards or HSA cards are all accepted methods of payment for services. When you schedule the initial visit, we request a credit card on file to hold the appointment for you. No charges will be applied to your credit card unless you miss or cancel an appointment without proper notice. On the day of your scheduled appointment, all charges for consultations, laboratory tests (Only for labs that say ‘Clinician billing’, Follow-up visit virtually, or in-person consultations will be billed to your credit card on file unless you provide other payment information and instructions prior to your appointment. Payment due at the time of appointment.If you are enrolled as Member, please check your Membership Agreement for details on your monthly fee, as payment is processed with automatic monthly membership billing.
Neither the RootHealthMD, nor the Practitioner Dr. Sailaja Reddy participate in any health insurance or HMO plans or panels, and has opted out of Medicare and Medicaid. Neither RootHealthMD, nor the Practitioner makes any representations whatsoever that any fees paid under this Agreement are covered by your health insurance or other third-party payment plans applicable to the Member. The Member shall retain full and complete responsibility for any such determination and payment of any fees. We cannot assist you with claim resolution. In addition, Dr. Reddy is not a Medicare provider. You will be provided with a service receipt. Dr. Reddy neither submit the patient’s medical notes to insurance companies nor assist in claim resolution.
Dr. Reddy does not fill out medical disability forms or medical necessity letters for patients.
Our office hours are Tuesday – Thursday, from 10 am to 5:30 pm Eastern time.
Our office may not be open during office hours as most of our visits are virtual.

PRESCRIPTION REFILL REQUESTS
As most of the prescriptions to our patients come from primary care physician. We request our patients to call primary care office for any refills. If you had received original prescription through us, we ask that you contact your pharmacy and have them fax over the medication refill request. Our fax number is (508) 452-0097. It may take up to 72 business hours to process a prescription refill. Please note that Dr. Reddy is generally not in the office on Mondays and Fridays to authorize refills. Please plan ahead to avoid any interruptions in your medications.
If you would like to schedule an appointment or cancel an appointment, have lab kit questions or administrative questions, please send a secure message via Portal.
Please note that it can take Dr. Reddy up to 48 hours to respond to emails.

Please refrain from wearing any perfumes, colognes, or heavily scented lotions to the office, as Dr. Reddy or our staff may be sensitive to these products.
